Imagine a lesson plan cites a specific page from a book, but you haven't provided that book and the system didn't actually find it. The complete book title and page number look credible, but they don't prove the citation is real.

Hallucinations can manifest as fabricated citations, incorrect facts, or judgments not supported by the materials. Not every unsuitable result is a hallucination: if a lesson plan's time arrangement deviates from requirements, it may primarily be a task understanding issue.

First Check Key Facts and Sources

For important names, dates, numbers, and quotations, go back to the original source to confirm. Check not only that the source itself exists, but also whether it supports the current claim. Even real webpages can be misread or cited for irrelevant content.

But having materials, using tools, or providing lengthy prompts cannot guarantee zero errors. Searches can mismatch, materials can be outdated, and the model can also misinterpret.

Don't Attribute All Problems to the Model

If a file hasn't been generated, first check the tool results; if the time arrangement exceeds 40 minutes, first compare against the requirements; if a citation doesn't exist, go back to the source. Accurately identifying the problem is key to choosing the right fix.
